The CDL/DOT physicals are performed by a DOT certified provider. The physical includes required components of blood pressure screening, vision screen, urinalysis, and physical exam.

Once you schedule an appointment, please keep the following things in mind before arriving for your appointment:

  • Urinalysis: You will be required to provide a urine specimen. Please be sure to drink plenty of water before your appointment.

  • Blood Pressure: Please avoid smoking one hour before your appointment as smoking can increase your blood pressure. Your blood pressure must be below 140/90 to pass your exam.

  • Heart Disease: If you have a history of heart attack, stents, angioplasty, or angina, please bring a clearance letter from your cardiologist, stating you can safely operate a commercial motor vehicle. You may also be required to provide a copy of exercise stress test and/or echocardiogram results completed within the last two years. 

  • Medications: Take all your regular medications on the day of your appointment.

  • Diabetes: If you have diabetes, please bring a copy of your A1C lab results from your primary care provider completed within the last three months.

  • Diabetes Treated with Insulin: If you take insulin for diabetes, please bring the “Insulin-Treated Diabetes Mellitus Assessment Form” (MCSA-5870) completed by your treating health care provider.

  • Sleep Apnea: Please bring a CPAP compliance report printed by your CPAP machine provider or sleep specialist that shows you are using your CPAP for a minimum of four hours nightly at least 70% of the nights. This report must show your CPAP use within the 30 days prior to your exam.

  • Stroke/TIA/Seizure/Brain Injury: Many individuals with a history of these neurological problems cannot qualify for a dDOT Medical Certificate. You may bring a statement from a neurologist indicating that you can safely operate a commercial motor vehicle.  This will be taken into consideration, but will not guarantee approval.
